When filling out the online form, please make sure that the form is filled out in its entirety and you are as detailed as possible when filling out your complaint information.   *Medicare patients need to select any and all aggravating conditions and conditions that affect your life and your ability to function*  Failure to do so may require form to be filled out again at the office.
